Requirements:  Current OR CNA license in good standing & CPR certification required. Able to complete 12 hrs of continuing education specific to Home Health/Hospice. Be able to work independently without constant supervision; react calmly & effectively in urgent situations; possess strong patient care & time management skills. Possess the ability to provide care for the patient’s age-specific, cultural, physical, and psychosocial well-being. Must possess quality writing, verbal & clerical/phone skills. Demonstrate strong personal integrity, stamina, problem solving skills & emotional stability. Be receptive to/in agreement w/hospice philosophy. Able to lift maximum of 75lbs w/frequent lifting of 50 lbs. Must have reliable vehicle with insurance coverage & current Oregon driver’s license. Maintain the care, use & safety of supplies/equipment & work areas in the proper performance of work duties. Establish/maintain effective working relationships w/patients, their family, peers, & staff. Able to prepare & maintain the patient records and reports. Must be computer literate & experience w/ mobile apps. Able to identify problems & recommend solutions. Ability to function in role without constant on-site supervision.
